MEDIA/MODEL RELEASE

I, ____________________________________am commissioning (and/or allowing) Melissa Loid Lazar, owner of The Whole Part Photography, (TWPP), Le Boudoir Photography, LLC, Le Boudoir Photographe, LLC and Le Portrait Photographer, LLC to photograph my child(ren) and or myself.

I understand that the photographs will remain the property of TWPP , Le Boudoir Photographe (y), llc, for an unspecified period of time, at which time they may or may not be deleted from the picture inventory.

Additionally, I understand that some, all or none of the photographs taken during my photo session may (will be) published to the photography website for my personal viewing and website promotions of Melissa Loid Lazar.

I have read and understand the above statements and I hereby give permission to Melissa Loid Lazar and her photography businesses to 1) photograph me and or my children and 2) grant permission to Melissa Loid Lazar to publish some, all, portions of or none of the resulting photo shoot photographs for public view and for promotional advertising purposes.

I agree to participate without financial remuneration, and I understand that this document releases Melissa Loid Lazar and the above named photography businesses from any future claims by me on behalf me or my children or any other claim resulting in an exchange of compensation, as well as from any and all liability, arising from the use of any photograph used in advertising.
